Avaira - Rebate Info
CooperVision - similar to other contact lens manufacturers - is offering a mail-in rebate when you purchase Avaira lenses.
For purchases made after January 1st, 2011
If you buy 4 boxes of CooperVision Avaira, your rebate is $15.
If you buy 8 boxes of CooperVision Avaira, your rebate is $35.
The 2011 rebate is available to new or existing lens wearers after getting an eye exam and purchasing Avaira contacts..
How to receive the Avaira Mail-in Rebate
Get an eye exam from your eye care practitioner.
Buy 4 or 8 boxes of Avaira contacts.
Complete the rebate process available on coopervision.com.
Attach the following:
- 4 end panels (What is this?) or 8 end-panels (for the $30 rebate) cut out from the boxes you purchased (see rebate form for instructions)
- A receipt of your eye exam (please circle the date)
- A receipt of the product purchase (please circle the date)

Some additional important notes:
- If you would like to check the status of your rebate, visit www.rebatetrack.com/coopervision (or call toll free 877-413-4692)
- Keep in mind that it can take 10-12 weeks for delivery of a Visa prepaid card. If card is not received within 12 weeks, call toll-free 877-413-4692.
- CooperVision will not accept submissions without an eye exam receipt.
- This offer is only valid for residents of the US and Virgin Islands.
- All purchases must be made within 60 days of eye exam.
- All receipts must be from the same prescribing practitioner or affiliated location. Rebate not valid in combination with purchase at Wal-Mart, Sam’s Club, or 1-800 Contacts.
- The fine print on the rebate form also mentions the following:
Offer not valid where prohibited by law. Not responsible for lost, late, illegible, stolen, or incomplete requests; or postage-due, damaged, or separated mail. Not valid with any other offer or rebates. Any obligation of CooperVision under this offer, and any check issued pursuant hereto, will expire and be null and void if check is not cashed or deposited within 60 days of issue. Offer good 1/1/11 to 06/30/11, must be postmarked by 08/29/2012. PLEASE NOTE: If the product you have purchased will be reimbursed by an insurance company or other third-party payor, reimbursement may not be sought for costs already covered by this rebate.
An end panel is a part of the contact lens box, that you need to cut out and attach to your rebate. It contains the base curve parameter, the diameter of the lens, and your prescribed power.
